For your convenience, all forms are electronic and HIPAA-compliant. Simply click the links below to complete your new client paperwork and necessary waivers. Once submitted, they are securely returned directly to us.
Our providers accept most major insurance plans. For those without insurance or who prefer to pay out of pocket, we offer highly competitive rates—$100 for a one-hour session.
Please note that we do not accept Medicaid.
