To make getting started simple, all new-client forms are electronic and fully HIPAA-compliant. Just click the links below to complete your paperwork and required waivers. Once submitted, your documents are securely transmitted directly to our office.
Our providers accept most major insurance plans. For clients without insurance or those who prefer to self-pay, we offer competitive rates—$100 for a one-hour session.
Please note that we do not accept Medicaid
